Get your teeth cleaned and examined every six months---

and we'll give you one kit of bleach FREE at each of your recall visits!!!

(1) Patient must have their teeth cleaned and examined first by Dr. Bonin, then every six months after that to be elgible.

(2) Patients are required to purchase upper and lower bleaching splints ($300.00), which includes the first box of bleaching medication (three 2.4 cc syringes of 16% Carbmide Peroxide).

(3) If oral health changes occur (broken, defective fillings or crowns, gum disease, soft tissue problems, etc.), recommendations to resolve and treat these issues will be made. Failure to accept recommended treatment options does not disqualify patients from this program, as long as they keep their regular scheduled cleaning appointments every six months. Other systemic diseases that arise may preclude the prescribing and usage of bleaching medications.

(4) If a patient needs to cancel or reschedule their regular six months cleaning and exam, they will have a two (2) month extension period which to reschedule and still remain elgible.  If the patient cancels or reschedules that appointment, they are no longer elgible for the free bleach.  After two (2) consecutive recall appointments, a patient may become elgible for the program again.

(5) Existing patients, who have already purchased bleaching splints, become elgible for this program at their next recall cleaning appointment.

(6) One kit of NiteWhite take home bleach contains three (3) 2.4 cc syringes of 16% Carbamide Peroxide. This is approximately equivalent to twelve (12) home bleachings if proper application and usage instructions are followed.  Bleach is only dispensed at the recall appointment and only to that patient.

(7) Additional bleach needed between six month visits is available but charged to the patient at its normal fee.

(8) Stronger concentrations and/or other preparations of take home bleaching medications are also available but will be sold at normal prices and are not included in this offer.

(9) This offer is non-transferrable.

(10) Offer only applies to patients of Dr. Todd Bonin, who have paid normal fees for dental cleanings, exams, and necessary x-rays, and also keep their recall cleaning/exam appointments on a regular basis as recommended by him and the hygiene staff.

(11) Other provisions, restrictions, or exclusions may apply.

(12) This program may be discontinued at any time and for any reason.
