** Patient must have teeth cleaned and examined every six months at Dr. Todd Bonin's office.  If oral health changes (broken, defective fillings or crowns, gum disease, soft tissue problems, etc.), these dental conditions will need to be resolved/treated before additional take home bleach is dispensed.  Other systemic diseases that arise may also prevent the regular use of take home bleaching medications.  One kit of NiteWhite take home bleach contains three (3) 2.4 cc syringes of 16% Carbamide Peroxide.  This is approximately equivalent to twelve (12) home bleachings if proper application and usage instructions are followed.  Stonger concentrations and/or other preparations of take home bleaching medications are available but will be sold at normal prices and are not included in this offer.  This offer is non-transferrable.
